How will the Patient Safety Reporting System work?

1. VA hospital staff members will fill out a form describing the incident.

On a removable section, the person filing the report will include his or

her name and phone number.

2. An analyst at NASA's Ames Research Center with medical expertise

will read the report and call the filer if there are any additional questions.

The analyst will then remove the section that identifies the person and

input the information into a database.

3. Staff members will then mine the data for patterns and issue advisories.
